This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review carefully.

Our commitment

Priority Rx is required by law to maintain the privacy of your protected health information ("PHI"), to provide you with this Notice of our legal duties and privacy practices, and to abide by the terms of the Notice currently in effect.

How we may use & disclose PHI

  • Treatment. To dispense prescriptions, coordinate with your prescribers, and provide pharmacist counseling.
  • Payment. To bill your health plan, verify benefits, and complete required approvals to dispense your medications.
  • Operations. For quality improvement, accreditation, training, audits, and business management.
  • Required by law. To respond to court orders, public health authorities, or law-enforcement requests as permitted by law.
  • With your authorization. Any other use or disclosure requires your written authorization, which you may revoke at any time.

Your rights

  • Request access to and copies of your PHI
  • Request corrections to your PHI
  • Request restrictions on certain uses or disclosures
  • Request confidential communications by alternative means
  • Receive an accounting of disclosures we've made
  • Receive a paper copy of this Notice on request
  • File a complaint with us or with the U.S. Department of Health and Human Services without retaliation

Marketing & sale of PHI

We will not use or disclose your PHI for marketing purposes or sell your PHI without your written authorization, except as permitted by law.

Breach notification

We will notify you in writing if a breach of your unsecured PHI occurs, as required by HIPAA.

Changes to this Notice

We reserve the right to change this Notice and to make the new terms effective for all PHI we maintain. Updated notices will be posted in our pharmacy and on this website.
